INTRODUCTION: arterial hypertension is the main cardiovascular risk factor. Adherence is challenged by socioeconomic factors; worldwide, only 21% of adults adhere correctly to their treatment. The study poses the following PIO format question: What are the main causes associated with non-adherence to antihypertensive treatment in patients over 40 years of age at the Primary Care Unit of the El Llano community, Baní, during February - March 2024? OBJECTIVES: to describe the causes of non-adherence to hypertension treatment; identify the age group with the highest adherence to hypertension treatment; evaluate adherence to pharmacological treatment for hypertension by sex; establish the relationship between non-adherence to hypertension treatment and educational level. METHODOLOGY: observational, prospective, cross-sectional, analytical-descriptive. The study will be reported following the STROBE guide criteria. RESULTS: patients over 60 years of age led adherence to treatment with 60%, and women constituted 59.12% of the total adherent population. Forgetfulness and economic limitations were the main barriers to adherence. Primary education was predominant among the most adherent patients. CONCLUSIONS: women over 60 years of age with primary education showed the highest adherence. RECOMMENDATION: to promote education on hypertension, family support, and access to medication.
